The giant leech is six feet long, several inches in diameter, and weighs close to four pounds. These large, slimy creatures are aggressive hunters. Unlike ordinary leeches that stay within water, giant leeches can travel in water or short distances on land. In either case, they use their sensory organs to detect changes in temperature and vibration, enabling them to detect and track prey up to 60 feet.
In combat, a giant leech attacks by latching its mouth onto prey. The initial attack requires a d20 roll. After a successful hit, the monster automatically inflicts 1d4 actual damage per turn as it drains blood from its victim.
The only way to remove a giant leech once it attaches is to kill it. Up to three giant leeches may attach to a single victim. When a giant leech consumes 15 hit points of blood, it detaches from its victim and leaves to digest its feast. Bloated leeches do not attack under any circumstance.
These creatures live in stagnant, murky water of swamps and marshlands in tropical or temperate climates. They are aggressive and feed on animals and humanoids alike.
